Non-Parent ID Card Request

This form is to request a Non-Parent ID Card for individuals or partner organization staff who are authorized by a parent or guardian to regularly pick up students from Yokohama International School.


A fee of 2,000 JPY per card applies to all issued non-parent ID cards. A link to complete payment will be provided after your request is submitted and reviewed by the school.


Requests must be submitted by a parent or guardian using the appropriate pathway below:


  • For Individuals (e.g., Grandparents, Nannies): Prior to completing this form, the individual must first be added as an authorized "Pick Up Contact" in the Veracross Parent Portal under "Contacts for [Child's Name]". Please note that any designated Pick Up Contact is also authorized to collect your child during emergency situations. Once added in Veracross, complete the form below to request their printed ID card.
  • For Companies / Organizations (e.g., After-School Programs): Submit the organization's primary contact details below. Upon receipt, YIS will reach out to the organization directly to issue our Partner Company Agreement and coordinate card issuance.
Photos should be a recent, clear, colour photo, facing the camera, with head and shoulders visible against a plain, solid-coloured background. Please use portrait orientation, leave some space around your head for cropping, and avoid hats, glare, shadows, screenshots, or photos of printed photographs. After cropping to a 3:4 portrait ratio, the image must be at least 354 × 472 pixels.
