Phone-Free Campus: Student Device Declaration

At YIS, we are committed to maintaining a focused, engaging, and phone-free learning environment for all Middle and High School students. To support this policy, each student who brings a mobile phone or smartwatch to campus is issued an individual locking pouch at the start of the school year.

Please complete this form if your child does not own a mobile phone/smartwatch, or will not bring one to campus under any circumstances (or to update us on a change of status).

By submitting this form, you acknowledge and agree to the following conditions:

• You confirm that your child will not bring a mobile phone or smartwatch to school or school-related on-campus activities.
• You understand that if your child’s status changes in the future, it is your responsibility as a parent/guardian to notify the school immediately so a pouch can be assigned.
• You understand that bringing an undeclared device to campus circumvents our shared safety and community agreements and will be followed up directly by school leadership.
