Yearbook Pickup​

Please fill out the following form to arrange to pickup your yearbook for your former student or to have it picked up by a current student.

Enter the details of the person picking up the yearbooks. We may ask for identification at the time of pickup.

FULL NAME REQUIRED. WE ARE UNABLE TO PASS ON YEARBOOKS WITHOUT IDENTIFICATION.

If a friend is picking up, please notify them directly. This form does not contact the pickup person.

By submitting, you acknowledge that YIS is not responsible for the yearbook once it has been picked up. Replacement copies for lost yearbooks may not be available.
