Temporary Guardianship (Out of Town Notice)


The Temporary Guardianship (Out of Town Notice) form is designed for parents or guardians to notify the school when they will be away from home for one night up to 7 days, ensuring the school has up-to-date contact information and details of a temporary guardian. Families must complete this form prior to their travel so the ES and MS/HS offices are notified.


This form provides essential information for the school to maintain communication and make necessary arrangements for the care of your child(ren) in your absence, including authorizing emergency treatment if required.


The Temporary Guardian must be at least 25 years old. It is strongly advised that temporary guardians are to be in place for no longer than 7 days and that this arrangement be in place only as a contingency to safeguard our students and to not be a frequent occurrence.


The child must reside with the temporary guardian during the parent(s) absence as there must be adult supervision where the child is residing.


Please complete the form below and a PDF form will be sent to both the parent and the temporary guardian via email for signature in the following days. An online signature by the parent and the temporary guardian is required for this process to be complete.
