Certificate of Enrollment Request Form

Please fill in the form below to request a certificate of enrollment for your child(ren). 

Please make sure that the information in your Household Profile is correct and up-to-date.
If you have changed addresses, please submit a Household Profile Update via your Parent Portal first. There may be delays in issuing a certificate of enrollment if the information in your Household Profile is inaccurate.

Please note that it may take up to one school week to process your request. If you are a family that is withdrawing, we may not be able to process your request until the last day of school for your child or until accounts have been settled. During school holidays, requests will not be processed.

Hard copies must be picked up from the Admissions Office during our office hours. Emails will be sent out once the request has been processed and the hard copy is available for pick-up. Hard copies will be retained up to two-weeks after completion. Any certificate not picked up after the two-week frame will be disposed and a new request must be submitted.
