Guest Student Permission Request

This form is to request permission for a child to attend Yokohama International School on a temporary basis as a guest of the school. Guest students are expected to follow all regular school rules and classroom procedures while at school.

Permission must be requested at least one week in advance by submitting this form to the relevant section principal.

Please complete the form below with basic information about the guest student and their host at YIS. Upon approval by the section principal, a Guest Student Agreement PDF will be sent to the parent/guardian email provided for digital signature. This agreement will request detailed health information, emergency contacts, and insurance details. The guest student may only attend once the signed agreement has been returned to the school.
